Position Title: Health Coordinator

Supervisor: Takoma Park Site Manager and MD Director
This position ensures that all health standards are met, provides health training for staff and parents, and monitors health records for children.
Essential Duties
- Work with the CentroNia Director and program staff to plan, coordinate, oversee, implement, and monitor a comprehensive health services program in accordance with MD Child Care License and other guidelines.
- Provide guidance to program and parents on children's health issues, including assessment of health and immunization status, identification of followup needs, communicable disease identification and notification, and access to community health care resources.
- Develop, implement, and update health and nutrition policies, plans, and procedures, and train staff and consultants.
- Improve monitoring systems for compliance with safety and health standards and licensing requirements.
- Identify, present, and promote current best practices related to infant and toddler health.
- Coordinate with community providers to ensure compliance with EPSDT standards and Healthy Kids screening.
- Coordinate and/or provide EPSDT screening to enrolled children.
- Evaluate and make referrals to local health providers for children and families. Document services and followup.
- Assist family support workers in linking families to an ongoing health care system.
- Conduct monthly health and safety inspections, including First Aid kits.
- Coordinate and provide staff training.
- Coordinate and/or conduct health assessments of children involved in the CCP Initiative.
- Assist child development and nutrition staff in conducting nutritional screenings and assessments, interpreting data, modifying menus, and accommodating special dietary requests.
- Work with CentroNia’s Food and Wellness department to integrate health and nutrition education activities into the classroom and home base curriculum.
- Provide and/or coordinate training for staff and parents on preventive health services, early intervention, breastfeeding, HIV, bloodborne pathogens, CPR, and First Aid.
- Provide ongoing training and technical assistance to staff and parents on prenatal and fetal development, postpartum issues, and typical and atypical infant and toddler development.
- Work with management staff to ensure completion of developmental, health, and nutrition screenings and followup on needs.
- Develop and maintain a resource library for health care information.
- Participate in interdisciplinary team meetings, case review meetings, and IFSP /EIP meetings.
- Participate in annual agencywide selfassessment and assist with corrective action plans.
- Establish and maintain an organized and accessible recordkeeping system.
Input health data into COPA information system.
- Prepare and provide timely and accurate written reports.
- Assist in the preparation for the Asthma and Allergy Friendly MSDE Certification and other certifications.
- Perform other duties as assigned by the CentroNia MD Senior Director.Required Experience and Education
- A Registered Nurse (RN) Program or a Bachelor’s degree from an accredited university in Nursing.
- A valid State license and two to five years’ experience working with infants, toddlers, and their families.
- Experience working effectively with families from various cultural and socioeconomic backgrounds, especially families with limited English proficiency.
- Bilingual proficiency in English and Spanish is required.
