  • * Job Summary:

Under the leadership of the Quality and Safety AVP, Director or Manager, the Patient Safety and Quality Specialist assists with coordination and implementation of the overall high reliability education and sustainment efforts to create and foster a culture of safety and quality among Wellstar facility team members, leaders, and physicians.

This role develops strong relationships with key leaders and collaborates with System and Facility Safety & Quality Staff to build and maintain a culture of safety, quality, and learning environment. This role is responsible for implementing and providing input regarding the evaluation of patient safety and clinical quality improvement programs. The Patient Safety and Quality Specialist, practices innovative design and systems thinking to improve measures of quality and safety. Supports robust quality improvement efforts and reduction of preventable harm within the facility.

Core Responsibilities and Essential Functions

Patient Safety and Quality Program Operations

  • Implementing and providing input regarding the evaluation of patient safety and clinical quality improvement programs.
  • Participates in training, education, and deployment of Safety-First program, including but not limited to high reliability leadership methods; high reliability (Error Prevention) tools; Fair and Just Culture; and Peer Reliability (Safety) Coaches, as needed.
  • Supports Wellstars Root Cause Analysis process including, but not limited to investigation; medical record review; research of literature and industry best practices; interviews; creation of events and causal factors map; supports identification of root causes, corrective actions, measures of success; and associated monitoring, tracking, and follow-up of all components of the RCA process, as needed.
  • Ensures timely investigation of patient safety issues. Supports the implementation of patient safety initiatives and harm reduction strategies, as needed.
  • Conducts and supports retrospective and/or concurrent quality and patient safety related chart reviews, as needed (i.e., Sepsis, Safety Learning System, etc.)
  • Supports and participates in review, analysis, reduction activities, and reporting of quality measures, including but not limited to Patient Safety Indicators (PSI), Healthcare Associated Conditions (HACs), and other measures as identified by the Centers for Medicare and Medicaid Services (CMS).
  • Supports the development of patient safety and quality related reports to department and system leadership.
  • Assists with Pre-Bill Audits/PSI review, as needed.
  • Promotes a culture of safety by participating in system-level Culture of Safety Survey activities.
  • Provides input and assists with facility action planning efforts using the culture of safety survey Results, as needed.
  • Coordinates the flow of data needed for the HEN.
  • Review and track insurance company quality concerns.
  • Reviews and updates policies and procedures.
  • Monitors core measures for the facility.

Robust Performance Improvement

  • Supports Quality Review Committee and Performance Improvement Committees at the facility level
  • Practices innovative design and systems thinking to improve measures of quality and safety. Supports robust process improvement and reduction of preventable harm within the facility.
  • Assists in the development of patient safety and quality measurement and improvement strategies.
  • Assists with the development of patient safety and quality reports for specific groups with Safety and Quality.
  • Supports hospital performance improvement strategies through problem assessment and education.
  • Supports data collection and analysis of patient safety and quality improvement trends to leaders as needed.

Regulatory Adherence and Compliance

  • Maintains current working knowledge related to applicable laws, regulations, statues, and best practice literature that affect the Patient Safety Program, including Georgia Medical Board, Board of Nursing, The Joint Commission (TJC), AHRQs National Quality Forum-Serious Reportable Events (NQF-SRE) and HIPAA.
  • Supports patient safety initiatives including, but not limited to sentinel event alerts, National Patient Safety Goals (NPSG), system goals.
  • Ensures adherence to regulatory standards.

Financial/Fiscal Responsibility

  • Demonstrates responsibility in financial management, continuous quality improvement, institutional support and representation, and customer service.

Required Minimum License(s) and Certification(s)

All certifications are required upon hire unless otherwise stated.

  • CPHQ - Cert Prof Healthcare Quality-Preferred

Required Minimum Experience

Minimum 2 years of experience in healthcare, nursing, clinical practice, or performance improvement. Required and

Experience in high reliability, performance improvement, and safety design principles Preferred
