Home Health Coordinator and field RN

Job Overview

We are seeking a dedicated and detail-oriented Health Coordinator to join our team. The ideal candidate will play a crucial role in managing patient intake, care plans, ensuring compliance with HIPAA regulations, and providing exceptional patient service. This position requires strong organizational skills and the ability to work effectively in a fast-paced home health setting. An RN with at least 1-2 years experience in home health preferred. Familiarity with axxess EMR preferred. Willingness to do home visits if needed preferred. If full time, max hours is 37.5 hours; Mon to Friday 09:00am to 5:00pm with 30 minutes break time. If part- time, needs to be willing to work 2-3 days a week from 09am to 5pm on working days. Must have evidence of ability to work, have an Illinois drivers license with un expired auto insurance. Volunteers are welcome. You must have active Driver's license, Auto insurance, proven evidence of ability to work in US, TB test or equivalent done within 12 months,

Responsibilities

Develop and manage comprehensive care plans for patients, ensuring all health needs are addressed.
Utilize Axxess software for patient data management, appointment scheduling, and documentation.
Maintain compliance with HIPAA regulations to protect patient confidentiality and sensitive information.
Provide excellent patient service by addressing inquiries, scheduling appointments, and assisting with medical collections as needed.
Perform clerical duties including filing, data entry, and maintaining organized patient records if needed.
Collaborate with healthcare providers to coordinate patient care and follow-up services.
Assist in the training of new staff members in home health office management practices.

Requirements

Experience working in a home health environment is preferred.
Familiarity with care plans and the ability to develop tailored health strategies for patients.
Proficiency in axxess electronic health records or similar electronic health record systems is highly desirable.
Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
Strong customer service skills with a focus on patient satisfaction.
Previous experience as a home health nurse or QA or coordinator is a plus.
Excellent attention to detail and accuracy in documentation.
Ability to manage multiple tasks efficiently while maintaining high-quality standards.
Willingness to work as field RN required.
